Home health is ripe for innovation

The current home health model relies on infrequent visits from skilled clinicians. Shifting to more frequent visits by home health aides has shown to reduce inpatient, outpatient, and ER usage for complex chronic patients.

RubyWell is helping forward-thinking, Medicare-certified agencies compliantly adopt this care model for chronic complex patients requiring longer lengths of stay.

About RubyWell

RubyWell helps families and care teams work together to safely manage health at home.

CMS claims data show that home health care improves patient outcomes and reduces healthcare costs. But only a fraction of eligible patients ever receive care. RubyWell is helping more Original Medicare patients access the care they’re entitled to though our home health agency partners.

Our proprietary technology and innovative approaches to providing covered home health services can help your agency expand the patient profiles you’re able to serve. This can grow your revenue, improve patient outcomes, save Medicare money, and ease some of the care responsibilities carried by unpaid family caregivers.

Home Health Agency FAQs

How does RubyWell find patients?

RubyWell reaches family caregivers of aging adults via advertising campaigns, organic content creation, and partnerships like NCOA and CAN. Our messaging shares information about the Medicare home health benefit and how we can help eligible patients access it. Families then choose to connect with us to learn more, check their loved one’s eligibility, and pursue care options.

What does the patient screening process look like?

Family caregivers and the homebound adults they care for are invited to complete a questionnaire at RubyWell.com. Their answers enable us to determine homebound status, insurance coverage, geographic location, health conditions, care needs, and hospitalization risk. Our proprietary Eligibility Engine can then predict if the patient is likely eligible for the Medicare home health benefit.

If a patient appears to be eligible, we schedule a call with them to get more details on health conditions and care needs, and to confirm Medicare coverage through a third party verification source. Once we have insurance verification, we send the patient a personalized Home Health Discussion Guide to share with their physician during their face-to-face encounter to discuss if a home health referral order is appropriate.

What is the Home Health Discussion Guide?

RubyWell has worked with clinical and regulatory partners to design a guide that captures all the information that:

  • A doctor needs to determine whether home health care is appropriate for a patient
  • Medicare requires to determine that the home health services being prescribed and delivered are medically necessary, and therefore covered

The RubyWell Eligibility Engine generates a personalized Home Health Discussion Guide for each patient that it predicts may be eligible for the Medicare home health benefit. We advise the patient and their family caregiver to share the Discussion Guide with the patient's physician during the face-to-face encounter to discuss home health. This allows patients to take control of their care, make informed decisions, and advocate for themselves.

The guide includes information about the patient's homebound status, hospitalization risk factors, and functional limitations. It also provides guidance for the physician in terms of what needs to be included in a compliant home health referral order, and instructions for submitting it for home health agency review. Each Discussion Guide features the logo and name of RubyWell’s partner home health agency.

How do you verify the patient's insurance?

We require the patient to share their Medicare ID number or photo of their Medicare ID card. We then run that information through a secure, third party insurance verification system called Stedi

How do you know if the patient is homebound?

Our questionnaire asks several questions regarding the patient's mobility and the level of assistance they need to leave their home. We also ask how often they leave the home, and if their doctor has advised them not to leave home due to their health condition. Our information is verified on a follow-up phone call with every patient.
