Take 3 minutes to help us predict if you or your loved one may be eligible for covered home health care

Why we ask for Medicare records
We review Medicare claims to:
Understand if Medicare is likely to approve and cover home health services based on the patient’s diagnoses, hospitalizations, ER visits, falls, etc.
Build a personalized Home Health Discussion Guide to facilitate the home health referral process

Medicare claims review
Our secure review process checks for:
Recent hospital stays or ER visits
This helps us understand recent changes in the patient’s condition and need for in-home support.
Current and past medications
This helps us understand what health conditions the patient is managing and whether ongoing care at home could be appropriate.
Diagnoses and medical history
Reviewing recent and long-term chronic health conditions helps us understand the patient's care needs.
What does this process look like?
- When the person needing care clicks the button below, they’ll log into a RubyWell account that we've already created for them. This allows them to share their Medicare records with us securely.
- Once we receive their Medicare records, we'll review them to understand their medical needs.
- We'll build a personalized Home Health Discussion Guide to share with their doctor when they meet to discuss if home health would be suitable. It can help the doctor write a referral order which our partner home health agency needs to onboard the patient.
We’re working closely with partner home health agencies on our Family Health Worker pilot program. This initiative will support family caregivers with certification and employment, making them eligible to become a member of their loved one's care team. If you care for someone who qualifies for covered care with one of our partners, you may be able to join our pilot program when it launches.

Trust isn’t given, it’s earned
RubyWell has been approved by the Center for Medicare & Medicaid Services (CMS) to safely and securely store Medicare claims data that beneficiaries share with us via the CMS Blue Button 2.0 technology. CMS approval is based on RubyWell’s compliance with their security and privacy policies.
Our company is committed to maintaining the highest standards of security and data protection. As part of this commitment, RubyWell also complies with HIPAA regulations and is SOC 2 Type 2 certified. You can review our SOC 2 Type 2 Report here.
RubyWell will never sell or share Medicare claims records with other parties.

RubyWell will have access to claims records for 13 months after they’re shared with us. Our access to Medicare claims records can be revoked by the person who shares them with us via their Medicare.gov account at any time.
Medicare guidelines require the Medicare beneficiary, or their Power of Attorney, to manage the sharing of any of their Medicare data because it is personal health information.
